Prior to your admission at the South Brisbane Day Hospital please ensure you have read the SBDH Patient Information Guide which outlines everything you need to know about your procedure at the South Brisbane Day Hospital. The booklet is yours to keep and does not have to be returned.
If you have been provided with physical forms, these need to be returned 5 business days before your surgery. Forms can be returned:

- via post to PO Box 8008, Woolloongabba QLD 4102.
